Sports registration forms are available in the front office, or you may download under the individual sport section. 

Information that appears on the form is important to Club programming and funding. We ask that every form is filled out completely so The Club can continue to keep youth sports affordable and consistent.

You may bring your completed form with payment to the front office or mail it. All forms must have original signatures.

As indicated on the Sports Registration Form, full payment is required to be submitted at the time of registration. Team spots will not be held for forms missing information, an original signature, or payment.

Resident fees apply to children living in Chelsea, Gardiner, Pittston, Randolph, Richmond, and West Gardiner. Nonresident fees apply to all other towns. For more information and cost please check each sports page.
